Your Patient Journey Step 2

What to Expect During Your Chiropractic Appointment

A chiropractic adjustment is often nothing like what first-time patients imagine. This page explains what actually happens during treatment — the sounds, the sensations, how your body may respond afterwards, and what is completely normal. Knowing what to expect helps you feel more comfortable and get more from your care.

What does a chiropractic adjustment feel like?

Most patients describe a chiropractic adjustment as a firm but comfortable pressure applied to a specific area of the spine or joint. The sensation is brief — the actual adjustment takes only a few seconds — and is followed by an immediate sense of release or relief in the surrounding muscles.

Some areas of the spine that have been restricted for a long time may feel tender during the adjustment, similar to pressing on a bruise. This is normal and passes quickly. Dr. Brian always checks in with you throughout the treatment and adjusts his approach based on your feedback.

What is the cracking or popping sound?

The sound is caused by a rapid change in pressure inside the synovial fluid of the joint — tiny gas bubbles form and collapse as the joint moves through its range. It is not bones cracking against each other, and it does not mean anything has been “put back in place.” See our full explanation of cracking sounds.

Research published in PLOS ONE (Kawchuk et al., 2015) used real-time MRI to confirm this mechanism. Importantly, the presence or absence of the sound does not determine whether the adjustment was effective.

Not every adjustment produces a sound, and that is completely normal. Effectiveness is not determined by whether a pop occurs.

Is it normal to feel sore after an adjustment?

Mild muscle soreness in the 24 to 48 hours after your first few appointments is common, especially if your spine and surrounding muscles have been restricted for some time. It is similar to the soreness you might feel after starting a new exercise programme — muscles that haven’t been moving freely are being asked to work again.

This soreness typically reduces significantly after the second or third visit as your body adapts. Applying a heat pack to the treated area for 15 to 20 minutes can help ease any post-treatment discomfort.

What should I do (and avoid) after my appointment?

Drink plenty of water after your visit — keeping well hydrated helps the muscles and connective tissue respond to treatment. Light activity such as walking is encouraged; prolonged sitting is not. Avoid heavy lifting, intense exercise, or activities that aggravate your symptoms in the 24 hours following treatment.

If Dr. Brian has given you specific exercises or stretches to do at home, doing these consistently between visits is one of the most important factors in how quickly you recover.

If you experience any new, sharp, or unusual symptoms after your adjustment — particularly numbness, tingling, or severe pain — contact the clinic promptly on (02) 9837 5161.

Are there techniques other than spinal manipulation?

Yes. Dr. Brian uses a range of techniques depending on your presentation, age, and preferences. These include instrument-assisted or soft-tissue techniques for patients who prefer a lower-force approach, dry needling, cupping therapy, remedial massage, and exercise rehabilitation. The Y-Strap adjustment is also available — a high-velocity cervical traction technique used for specific presentations.

Paediatric adjustments use very gentle, low-load techniques appropriate for children of all ages, including infants. No two treatment plans are identical.

Common Questions

Adjustment FAQ

Does a chiropractic adjustment hurt?

For most patients, no. You may feel brief pressure or mild discomfort during the adjustment, particularly in areas that have been restricted for a long time. Dr. Brian always checks in with you throughout treatment and will modify his technique if you are uncomfortable.

How long does a return chiropractic appointment take?

Return appointments are typically 15 minutes. This includes a brief check-in on how you have been feeling since your last visit, any reassessment needed, hands-on treatment, and any exercises or advice for between visits.

Can I have a chiropractic adjustment if I am pregnant?

Yes. Dr. Brian is experienced in prenatal chiropractic care and uses modified, pregnancy-safe techniques throughout all trimesters. Many pregnant patients find chiropractic helpful for lower back pain, pelvic discomfort, and postural changes associated with pregnancy. Always let Dr. Brian know you are pregnant at booking.

Is chiropractic treatment safe?

Chiropractic care has a strong safety record when performed by a registered practitioner. Serious adverse events are rare. For a full evidence-based overview of chiropractic safety, see our Is Chiropractic Safe? page.
